چکیده

Tumour necrosis factor (TNF)-a is currently being targeted in autoimmune joint diseases, including rheumatoid arthritis (RA) and spondylarthropathies [1, 2]. Widespread use of antiTNF-a therapies (e.g. etanercept) has unveiled a spectrum of adverse effects, including infection [3] and a systemic lupus erythematosus (SLE)-like syndrome [4]. The latter appears to be biologically based on perturbations in TNF-a-controlled homeostasis of interferon (IFN)-a [5], and is betrayed by seroconversion of SLE markers [6]. With interest, we read the occurrence of pleural effusions associated with anti-TNF-a agents [7]. We encountered a patient with RA taking etanercept, who developed alternating pleural effusions. We obtained measurements that suggested that a lupus-like syndrome and not a common drug reaction precipitated these pleural effusions.

Etanercept-induced lupus accompanied by hemophagocytic syndrome.

Hemophagocytic syndrome (HPS) is a severe, potentially life-threatening disorder characterized by an excessive activation of macrophages, such as may occur in the setting of lupus. A 62-year-old Japanese woman treated with etanercept for rheumatoid arthritis developed persistent fever, cytopenia, coagulopathy, and hyperferritinemia. Simultaneously, lupus-like features including pleuritis, hypoc...

Successful Etanercept Therapy for Refractory Sacroiliitis in a Patient with Ankylosing Spondylitis and Mixed Connective Tissue Disease

The concurrence of ankylosing spondylitis (AS) in a patient with mixed connective tissue disease (MCTD) is rarely described in the literature. Significant and sustained efficacy with tumor necrosis factor (TNF)-alpha blockers has been demonstrated in AS patients. However, evidence to date has revealed associated side effects, including antinuclear antibody induction and development of a lupus-l...

Systemic lupus erythematosus induced by anti-tumour necrosis factor alpha therapy: a French national survey

The development of drug-induced lupus remains a matter of concern in patients treated with anti-tumour necrosis factor (TNF) alpha. The incidence of such adverse effects is unknown. We undertook a retrospective national study to analyse such patients. Between June and October 2003, 866 rheumatology and internal medicine practitioners from all French hospital centres prescribing anti-TNF in rheu...

Spontaneous resolution of lupus nephritis following withdrawal of etanercept.

INTRODUCTION Etanercept, a systemic inhibitor of α-TNF, is used for treatment of various autoimmune disorders. We report a case of spontaneous resolution of etanercept-induced lupus nephritis. CASE DESCRIPTION A 57-year-old female patient taking etanercept for psoriasis presented with laboratory-and histology-confirmed lupus nephritis. After stopping etanercept, there was normalization of pro...
